Complex conditions deserve careful, coordinated, non-rushed care.
Living with chronic conditions like high blood pressure, diabetes, heart disease, or autoimmune illness shouldn’t mean juggling a dozen medications and specialists on your own. At 3 Roots Concierge Medicine, we slow down enough to understand the whole story and walk with you as your health evolves.
In traditional settings, chronic disease care often looks like quick medication refills and fragmented advice. In our concierge model, you have:
A physician who knows your full history.
Enough time to think together, not just react.
Help organizing and interpreting what every specialist says.
You’re not “too complicated.” You’re exactly who this model was designed for.
Making sure cardiology, endocrinology, rheumatology, and other specialists all align with your big-picture plan.
Looking at trends over time, not just isolated lab results.
When needed, we dig into the latest evidence and connect with experts to ensure your care is up to date.
Minimizing drug interactions and reducing unnecessary medications whenever safely possible.
We see you more often when needed, then adjust as your condition stabilizes.
From clarifying what happened to coordinating appropriate follow-up, you don’t have to navigate it alone.

We follow evidence-based guidelines—but not blindly. Your preferences, your daily life, and your comfort level matter. Together, we:
Set realistic goals.
Choose treatments you’re more likely to stick with.
Adjust over time as your body and circumstances change.
If your care feels fragmented, rushed, or confusing, a concierge relationship can make chronic disease management feel more coordinated and less overwhelming.
We frequently care for people with high blood pressure, high cholesterol, diabetes and prediabetes, heart disease, thyroid disorders, autoimmune conditions, and more. If you’re unsure, we can discuss your specific situation in a meet & greet.
Not necessarily. In many cases, we continue working with your specialists—but with better coordination, communication, and advocacy so the care feels connected instead of scattered.
Often, yes. We carefully review your medications, look for interactions, and see where we can safely streamline. The goal is effective treatment with as little unnecessary complexity as possible.
It depends on your conditions and stability. Early on, visits might be more frequent. As things stabilize, we’ll space them out—but you still have access between visits for questions and concerns.
Yes. Chronic disease management is a core part of concierge primary care at 3 Roots and is included in membership. Certain tests, imaging, or specialty visits may still go through your insurance.
